The title page should include the full title of the manuscript and name(s) of author(s), along with degrees/titles/affiliations. A complete mailing address, daytime telephone and fax numbers, and e-mail address should be included for the designated corresponding author. References should be provided for all statements concerning specific published data. Recent references (within the past five years) are preferred.

Authors are encouraged to provide illustrations (eg, diagrams, charts) to accompany manuscripts. Rough sketches may be submitted for redrawing by our illustrator. A descriptive legend should be provided for each illustration. Please label illustrations with author and title of manuscript and indicate top of image with arrow.
Illustrations, tables, or quotations to be reprinted from other publications should be accompanied by a complete citation of the source. Permission to reprint will be obtained from the copyright owner. A signed release is required for all photographs. Identifying details should be omitted in any patient histories.

PEER REVIEW:
Submissions are sent for review to members of the Editorial Board, who may accept, reject, or recommend revision. Authors are notified of the Editorial Board's decision in writing. Upon acceptance of the manuscript and receipt of a signed copyright assignment form from each author, manuscripts become the property of the CMSC and RIMS.

COPY EDITING:
All manuscripts will be edited for clarity, style, conciseness, and format. The edited manuscript will be sent to the designated corresponding author for approval. The corresponding author is responsible for communicating with coauthors. Authors may be asked to answer editorial queries or to implement revisions.
