CMSC NEW MEMBERSHIP APPLICATION
All membership applications are reviewed by the CMSC Membership
Committee and require the submission of some basic information in
addition to your professional qualifications. If
you are interested in becoming a CMSC member, please choose the
appropriate form below, complete and return to the CMSC administrative
office.

CMSC MEMBERSHIP CATEGORIES
Full Membership
To
qualify for full membership, a center must be engaged in the
comprehensive care of persons with multiple sclerosis and research
related to the disease. There must be clear evidence of comprehensive
care, including the ability to evaluate, manage medical, psychological,
social and rehabilitative problems related to MS. In addition, full
members must have specific educational programs for patients,
caregivers, and professionals. Full members must also be active in
research related to the problems posed by MS.
Associate Membership
Associate members are health care providers who do not meet the
criteria for full membership, but are involved in the comprehensive care
of patients with multiple sclerosis.
VA Members
VA members must be clinics or centers that are part of the VA system and provide.
Their programs must meet all requirements for full membership.
Liaisons
Liaisons are corporations, organizations, and other groups not directly
involved in with MS care or services but wish to support and be
affiliated with the CMSC.
Individual Membership
Individual members include professionals (physicians, health
professionals, scientists, students) who wish to associate with the
Consortium and its programs. These members must be engaged in the care
of persons with multiple sclerosis or their care givers or have a
special interest in MS research. This category includes those providers
and clinics that are too limited in their scope to qualify for full
membership. Student Members must be full-time students in the health
care field or related discipline and have a special interest in MS.
